Welcome to the Pier School Clinic Corner. I hope these web pages help provide you with useful information and answers to many of your questions. Please contact me with any changes in your child’s health or concerns you might have that would impact them in school. It is also important to inform the school of any changes in your child’s emergency contacts numbers.

Parents are advised to check the immunization requirements for their child’s grade level as changes in state regulations have increased the required vaccinations. 7th grade students are also required to have a report of a recent physical.

All student athletes are required to have a physical within the year of their sport’s tryout date. Please send in the necessary paper work as soon as possible or contact me with any questions or if you need assistance in obtaining services.

Immunization to protect your children from preventable diseases

According to the R.I. Department of Health

5th grade students should have had:

  • 4-5* DTP/DT (Diphtheria, Tetanus Pertussis) *depends on age when series was started
  • 4 Polio
  • 3 Hepatitis B (series of 3)
  • 2 MMR (Measles, Mumps, Rubella)
  • 2 Varicella (Chicken Pox) or MD notation of Disease

6th Grade students should have had:

  • 4-5* DTP/DT
  • 4 Polio
  • 3 Hepatitis B (series of 3)
  • 2 MMR
  • 2 Varicella (Chicken Pox) or MD notation of Disease

7th Grade students should have had:

  • A report of a recent Physical
  • 4-5 DTP/DT/Td
  • 4 Polio
  • 3 Hepatitis B (series of 3)
  • 2 MMR
  • 2 Varicella (Chicken Pox) or MD notation of Disease
  • 1 Meningitis Vaccine

Medication Policy

Medication Forms are available online and in the office.
Prescription and non-prescription medication will be dispensed by the school nurse-teacher only when:

  1. received by the school nurse in the original labeled container from the pharmacy or manufacturer;
  2. authorized in writing by the parent or legal guardian;
  3. requested in writing by the attending physician.

At the Middle school and High school level Tylenol (Acetaminophen) in a dose appropriate to age and weight may be administered by the school nurse with parent permission. The parent/ guardian will be notified of the student’s complaint and time given.

Screenings help detect problems for early care and treatment

Vision Screening is conducted by the school nurse for 5th and 7th grade students each year. However, any student can be screened if the parent, student or teacher has concerns about their vision. Parents will be notified by mail of any failure and the student should be evaluated by their eye physician.

Dental Screening is conducted by Donna Hagerty, our school dentist for the 5th grade students in the fall. Students who have brought in reports from their own dentist, are exempt. If indicated a referral will be mailed to the parents.

Scoliosis Screening is conducted by the school nurse for 6th, 7th and 8th grade students in the spring. Scoliosis is a lateral or sideways curvature of the spine with onset often occurring during the adolescent growth spurt. If detected early, many cases can be controlled. A referral for an evaluation by your doctor will be mailed to parents if indicated.

The goal of each screening is early detection of a problem so that it can be treated and corrected. If you do not want your child to be screened please notify Mrs. Fair.
