UPDATED FORM
RIPCOA recently updated the Assumption of Risk form. This form's purpose is to be prepared to move forward with sports in school. ALL potential athletes must complete this form and have notarized regardless of grade. The form can be found HERE. Once complete and notarized, please turn your completed paperwork to the front office or email to Michael Millen at [email protected].
